CDOG requires your pet to have a rabies vaccine, some form of distemper/parvo vaccine, and a yearly fecal exam (with negative results). We strongly recommend having your animal vaccinated against bordetella and on heartworm, flea, and tick prevention.
We are asking you include copies of the vet paperwork with the required vaccines/exams highlighted along with the completed form. We do not need all of your dog's paperwork; just the sheets that have the vaccination information and date on them. Please be sure to include you vet's name and phone number as well.
This health form can be obtained from and turned in to any officer at the meetings, training sessions, or social events but they must be received before your dog can participate in any activities.
